To enroll in District 191, complete the following forms and return them to the Enrollment Center.
Packets can be emailed, dropped off in person or faxed to Enrollment Center.

Enrollment Checklist
- Immunization Form—Make sure your child's immunizations are up-to-date before school starts—download the immunization form. Learn more about immunizations in District 191.
- Proof of Child's Age—Proof of your child's age may include a copy of a birth certificate, I-94, passport, hospital record, baptism record, or other official government issued document. The document is used to verify date of birth and also the student’s legal name. Students must be 5-years old on or before September 1 to register for kindergarten. Early entrance into kindergarten may be requested if the child was born in September or October and meets qualification criteria.
- Proof of Residency—Please include a copy of a current utility (gas, electric, cable, telephone, or water) bill, rental or lease agreement, home purchase offer (closing paperwork), or home tax bill.
- Early Childhood Screening (applicable to kindergarten enrollment only)—Screening is a free and simple check ouf your child's vision, hearing, height, weight and development (thinking, communication and language, large and small muscles, and social/emotional development), as well as a brief review of health history and immunizations.
Application for Educational Benefits
The Application for Educational Benefits determines if families qualify for free or reduced-price school meals. If a family qualifies for either, children may also have a free breakfast. The district receives additional funding based upon the number of qualifying families. We encourage every family to either complete or waive the Application for Educational Benefits.
