Child Care Information
Independent School District 191 endeavors to better serve parents and students by gathering accurate information about students who will attend before- or after-school child care facilities. The information you provide on the Student Child Care Information form is important in several ways:
1. It provides our Transportation Department with accurate counts as to the number of students who will actually ride the school bus and how many students can be expected at each bus stop. These counts help to plan efficient use of our buses. Transportation will be provided only if the child care facility is located within each school’s attendance area.
2. In the event that an emergency would occur on a particular bus, your school and the Transportation Department will have access to relevant information.
3. If your child will attend a child care facility such as Project K.I.D.S., Kindercare, or any other individual provider, before or after school, any day of the school week during the school year, please complete the CHILD CARE INFORMATION form below and return it to your school or to Student Transportation Services, ISD 191, 200 West Burnsville Parkway, Burnsville, MN 55337-1613. (For the 2017-18 school year, please submit this information by June 23, 2017.) The form is available for download below or can be submitted online. Please be sure to provide complete address information including the address of the child care provider. The information you provide will be considered confidential and will be used for internal school district purposes only.
Students eligible for transportation to and/or from their child care facility will then be included in bus routes for the first day of school.
For the safety of all students, one address for pick up and one address for drop off will be accommodated.
